Medical Form

This is the newest 2010 Medical Form that is required for all scouts and adults. Updated 3/30/10. There is a new Medical Form Section D on this web site, which is also required by the State of Connecticut. Any medication that is dispensed at camp must also have the Medication Authorization form.
